Getting Help

The Shelter Referral Flow chart below provides a brief overview of the shelter referral process.

Preview only. Fonts, colors, and box styling approximate your template; the real page will use OpenCities' own styles.

The steps below outline the shelter referral process.

  1. Outreach

Client contacts and/or engages with Outreach Teams: Familiar Faces, Tracy Community Connections Center or Sutter Tracy Community Hospital

  2. Assessment

The Outreach Team will assess the needs of the client and identify resources that may support the success of the client. This may or may not include a shelter intake.

    Assessment

The Outreach Team will complete the shelter referral form and background check (clients will not be permitted to the program if they have active felony warrants, arson, or 290 registrants). Once background is cleared the referral will be emailed to Shelter Operator case management team. Please take into consideration that the teams do not work 24/7. The shelter will respond to confirm request has been received and the referral has been added to the waitlist.

    Denial

The Outreach Team will continue to provide case management services and provide additional resources and support to the unhoused individual.

  3. Opening at the shelter

Shelter Operator will notify the referring Outreach Team upon completion of background check prior to the client being scheduled for intake. If the client does not show up and/or refuses services, Shelter Operator will place the client on the bottom of the waitlist and notify the outreach team member who made the referral. Outreach teams will continue to provide case management and engagement.

  4. Intake Process

Shelter Operator will work with the client on their intake forms and prepare them for their stay at The Tracy Shelter Program. This includes proper assessment of their current needs and barriers, proper handling of their possessions and supporting them with familiarity of the shelter facility. If applicable, this will also include any support needed for a pet if they are with a pet upon intake.

  5. Enrollment in the shelter

Shelter team is responsible for shelter intake, case management of shelter clients while enrolled in shelter program and oversight of shelter facility. The shelter will also provide 3 meals a day, onsite laundry services, day programming and transportation support.

  6. Leaving the shelter

    Positive exit

    • Permanent housing
    • Transitional housing
    • Family reunification

    Client exit

If client is exited from the shelter, the referring team will be notified, and case management/engagement services can restart through outreach teams.

Tracy Police Department’s Familiar Faces Homeless Outreach Team supports the City of Tracy’s Strategic Plan to Prevent and End Homelessness by serving as an initial call, triage level, and follow-up response team to identify, engage, and assist those at-risk of or experiencing homelessness in the community. The team provides resources about mental health, substance use disorder treatment, motivational interviewing, trauma-informed care, harm reduction integrated care, and care coordination, through utilizing community, county, and state resources to help divert unsheltered individuals off the streets by offering transportation services to individuals in need of reunification, shelter, urgent care services, and housing options, including the City’s Temporary Housing Shelter. Familiar Faces also provides case management services to those that do not qualify for the Tracy Interim Shelter.
